After several specialist visits, I would worry about the loose ends: who is following up on what? I would start with one page.
Three columns: which clinician handles which issue, the medicines I actually take, and the tests or appointments still pending. Beside each pending item I would put a date and a contact person. An unknown would get a question mark to bring to the visit.
I would take that page and visit summaries to my primary care doctor. If medication instructions conflicted, I would ask the clinicians or pharmacist to clarify before choosing a version myself.
It would not need to look like homework ready to hand in. Legible enough to help the next conversation would do.
